Accord Housing runs Be Well Community Offer, a social prescription service for people living within Tipton, Princess End and Great Bridge. The service is free of charge and can be accessed by anyone over the age of 65 who has a need for community based support. Be Well covers a variety of areas including supporting access to; • Social opportunities • Carer support • Home Adaptations/ Telecare • Plan for hospital discharge • Meals/ nutritional support • Falls prevention / walk confidence • Information/ advise on specific health conditions • Befriending Ultimately Be Well aims to reduce the need for unnecessary hospital admissions and social care therefore our service will work with individuals to achieve this.
